What information is necessary for requesting the Certificate of Analysis (COA)?

Information for the COA application includes:

1. Title of sample
2. Appearance of sample
3. Manufacturer’s name
4. Manufacturer’s address
5. Exporter’s name
6. Exporter’s address
7. number and PI number
8. Description of product and container
9. Quantity of the export product

And the COA is valid for only the lot of product indicated in the COA request form.
